UniCare Now Offering Innovative Program to Reduce Hospital Admissions and Improve Health Outcomes for Older Adults
ANDOVER, MASS. — UniCare is now offering an innovative, at-home solution to manage chronic conditions as part of its continuing efforts to improve the health of its members by simplifying the healthcare experience. Through its partnership with Senscio Systems, UniCare retiree members will have access to a new virtual chronic care management program called Ibis Health that supports each individual’s whole health through daily connected care, round-the-clock monitoring, and one-to-one monthly support. UniCare, which administers health benefits for nearly 200,000 Massachusetts state and municipal workers and retirees, will offer this to its Medicare Extension Plan members.

Individuals who qualify will have the opportunity to participate in this virtual-care program that turns the home into the first point of access for day-to-day healthcare. Members receive personal telehealth tools for convenient self-management of their conditions. To date, Ibis users, who are often older adults battling chronic conditions such as COPD, diabetes, and heart disease, are reporting 37 percent fewer hospital admissions.
